Hesitation in accelerating after engine is warm

I find that when I accelerate straight after changing gear the car tends to hesitate before pulling away. At first i thought clutch but foot is completely off the pedal. Also I keep my foot at the same position with the accelerator so it feels as if the car "waits" before picking up speed.

Strangley it only happens when the car is warm. Flys along when i first pull out of the drive.

Is this what is known as a "flat spot"?

I also find it can be lumpy in idle, not sure if this is related...
